1. What are our purposes for processing your personal data and which personal data we process?

Communication Activities

If you fill in the contact form on our website, some of your personal data may be processed. In this context, if you request information about the medical services we provide by contacting us, your information will be processed in order to determine whether the Service you request is within the scope of the services we provide, to carry out the information and communication processes related to you, if your communication purpose is a suggestion and a complaint, to provide you with the most appropriate conclusion of this purpose, to create an appointment if you request, to arrange your appropriate appointment period according to the Service you request, to communicate with you in case of possible situations related to your appointment (change, postponement, cancellation, etc.), identity (name, surname), communication (phone number, e-mail address), customer transaction (contact subject, type of treatment you request, purpose of communication, notes you will send to us, appointment date and time).

Legal Processes

In order to use all kinds of lawsuits, responses, demands and objection rights and obligations against official institutions and organizations such as courts, enforcement offices, Personal Data Protection Authority, Ministry of Health, finance, conducting negotiations and negotiations on disputes in possible disputes that may arise between us, in case of a possible complaint/investigation, your identity (name surname), communication (phone number, e-mail address), customer transaction (date of disclosure, channel, method, scope and related records) will be processed in order to prove the illumination made in accordance with the Law.

• Suppliers: Service providers to receive product and service support for information technologies or consultancy requiring expertise, etc. (suppliers from whom maintenance/repair/development service is received, suppliers from whom server service is received regarding the website, legal consultancy service providers, e-mail server service providers, suppliers providing patient tracking and management system service),



• Public Institutions: With these institutions, organizations and authorities mentioned the information requested from us in order to fulfill our information, documents and other related obligations against authorized public institutions and organizations and judicial authorities and to use our legal rights such as lawsuit and response rights.

3. What is the method and legal reasons for collecting your personal data?
